Namespace import path under "/otel" (#274) 
Also fixes example module paths so that they use the vanity URL instead
of the github URL.

* rename package tag to package distributedContext

* rename Tag -> Entry

* change distributed context -> dctx to avoid capitalization collision

* change dctx -> distributedcontext

* update stackdriver client example
