We don't currently support values 1 (centimeters), 2 (inches) or 3 (DAR),
only the default value 0 (pixels) which doesn't need to be written.
The fate refs are updated as unknown SAR is now signaled in the output
files with the addition of the new element.

This allows to copy information related to the stream ID from the demuxer
to the muxer, thus allowing for example to retain information related to
synchronous and asynchronous KLV data packets. This information is used
in the muxer when remuxing to distinguish the two kind of packets (if the
information is lacking, data packets are considered synchronous).
The fate reference changes are due to the use of
av_packet_merge_side_data(), which increases the size of the output
packet size, since side data is merged into the packet data.
This is never mentioned in the specifications, and decoders work
just as fine without it. Update the fate references since the compressed
file is smaller.

Also support disabling them as they seem to cause problems to some
Users. They are also not allowed in IRT D-10 thus the default for
mxf_d10 is not to write them
This also decreases the filesize when no user comment are stored

Compute individual stream durations in matroska muxer.
Write them as string tags in the same format as mkvmerge tool does.

The old one is the result of the reverse engineering and guesswork.
The new one has been written following the now-available specification.
This work is part of Outreach Program for Women Summer 2014 activities
for the Libav project.
The fate references had to be changed because the old demuxer truncates
the last frame in some cases, the new one handles it properly.
The seek-test reference is changed because seeking works differently
in the new demuxer. When seeking, the packet is not read from the stream
directly, but it is rather constructed by the demuxer. That is why
position is -1 now in the reference.

The mp2 seek test results change. Whether to skip samples if the file
had no LAME gapless tags was inconsistent. When seeking to the start
of the file, 529 samples were skipped, but when playing from start,
nothing was skipped. This commit changes the behavior on seek to skip
nothing.

Normally, a Laplace distribution is more typical of the residuals
encoded, but for noisy input, it's both better and simpler to be
safe and use a 1/d^2 distribution. Second hunk could use some
renormalization but it has effectively little impact.

It has not been properly maintained for years and there is little hope
of that changing in the future.
It appears simpler to write a new replacement from scratch than
unbreaking it.
adpcm_ima_qt does not produce reproducible results, so it is temporarily
disabled (see #3701).

This results in DefaultDuration not being written when the framerate is
not known, but as this field is purely informative, this should not
break any sane demuxers.
