Fixes an issue with one output channel being slightly louder than
the other. The output now matches other public HCA decoders.

I suspect this was originally broken by b7e5c8f , but even
then, it only worked because it read out of bounds from
intensity_ratio_table.

This causes indexes into scale_conversion_table to wrap around, alternatively they
could be clipped, the table be enlarged or we can error out. I have not found a document that specifies
what is the correct way to handle this
