Dave YeoandLuca Barbato 4da585ae47 OS/2: Cleanup slib_create_def_cmd
Export symbols by name rather then ordinal.
Remove PROTMODE directive as it does not make sense for 32 bit library.
Also silences a warning from some linkers.

Dave YeoandDiego Biurrun 9c167914a1 x86: Fix assembly with NASM
Unlike YASM, NASM only looks for include files in the current
directory, not in the directory that included files reside in.

Dave YeoandDiego Biurrun 197439c1ef x86: pngdsp: Fix assembly for OS/2

Dave YeoandMans Rullgard b58b9fa3b5 Make sure kbhit() is in conio.h
Conio.h is a non-standard header and may not have kbhit()
prototyped. This fixes compile on OS/2 where the EMX version (we're
using a fork) of conio.h only has getch() and getche().
