Use the newly created vlc.h directly instead of including get_bits when needed.
The VLC and RL_VLC_ELEM structures are independent from the bitreader.

Alexandra HájkováandJanne Grunau aebf07075f dca: change the core to work with integer coefficients. 
The DCA core decoder converts integer coefficients read from the
bitstream to floats just after reading them (along with dequantization).
All the other steps of the audio reconstruction are done with floats
which makes the output for the DTS lossless extension (XLL)
actually lossy.
This patch changes the DCA core to work with integer coefficients
until QMF. At this point the integer coefficients are converted to floats.
The coefficients for the LFE channel (lfe_data) are not touched.
This is the first step for the really lossless XLL decoding.

Alexandra HájkováandLuca Barbato 8375dc1dd1 asfdec: handle the case when the stream index has an invalid value better 
The demuxer returned INVALIDDATA and failed to demux the remaining data
when an invalid stream index was read, now it just skips the asf packet
for the stream with an invalid stream index and continues demuxing.

Alexandra HájkováandAnton Khirnov b08569a239 lavf: Replace the ASF demuxer 
The old one is the result of the reverse engineering and guesswork.
The new one has been written following the now-available specification.

This work is part of Outreach Program for Women Summer 2014 activities
for the Libav project.

The fate references had to be changed because the old demuxer truncates
the last frame in some cases, the new one handles it properly.
The seek-test reference is changed because seeking works differently
in the new demuxer. When seeking, the packet is not read from the stream
directly, but it is rather constructed by the demuxer. That is why
position is -1 now in the reference.
