Revert memory mapped support for LOD files (r881, r886 and r895) because on Windows these files get locked and other applications cannot access them anymore.

Because of really huge amount of modifications here I've probably missed some minor stuff. I doubt if it's still compatible with gcc - some changes should be reapplied.
